Position Overview

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across multiple states. The Care Coordinator, Embedded provides on‑site support within partner long‑term care facilities in the Ashburn, VA area. As an integral, in‑person member of the clinical care team, you work alongside providers in a family‑practice‑style model that emphasizes continuity, relationship‑building, and patient‑centered support.

Key Responsibilities

  • Coordinate care for long‑term care residents using Ennoble Care’s EMR
  • Maintain regular, proactive on‑site engagement with residents to support care plans, follow‑ups, and care transitions
  • Collaborate closely with Ennoble Care providers within the long‑term care setting
  • Serve as a key liaison between providers, facility staff, residents, and families
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
  • Accurately document patient interactions and care coordination activities
  • Follow established workflows, documentation standards, and escalation protocols
  • Contribute positively to team collaboration and strong facility partnerships

Requirements

  • Legally authorized to work in the United States
  • Active, unrestricted clinical certification or licensure (e.g., MA/CMA, LPN, RN)
  • Minimum one year patient‑facing experience in long‑term care, skilled nursing, or similar clinical setting
  • Comfortable communicating via Microsoft Teams, email, phone, and in person
  • Positive, approachable relationships with residents, families, providers, and facility staff
  • Strong organizational and time‑management skills with an optimistic, initiative‑driven mindset
  • Working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365 (Outlook, Word, Excel)
  • Full‑time availability Monday through Friday, 8:00 a.m.–4:30 p.m. EST
  • No on‑call; daytime hours
  • Seven company‑paid holidays annually
  • Valid driver’s license and reliable transportation to meet job duties (must provide proof of current auto insurance)
  • Ability to pass a background check and possible travel between facilities
