Job Summary
Location:
Ashby Ponds by Erickson Senior Living. The Wellness Nurse monitors residents' health and wellness, ensuring overall well‑being, providing training, maintaining documentation, and collaborating with team members.
2‑week repeating schedule:
Week1 – Sunday & Wednesday;
Week2 – Tuesday & Saturday, 8:00am‑6:30pm, with 30‑minute unpaid break.
Hourly $33–$37, with shift differentials: weekday evening $2.00, weekday night $2.50, weekend $4.00.

- Develop relationships with residents and families, placing residents first.
- Complete observations and evaluations to identify changes in clinical condition.
- Update or modify residents' service plans as needed.
- Ensure medical needs are met, monitoring medication profiles for accuracy and timely documentation.
- Reconcile medication deliveries.
- Monitor residents after falls, accidents, or incidents.
- Collaborate with Assisted Living, Memory Care, and Wellness Managers to analyze outcomes.
- Assist in coordinating ancillary support services (home health, hospice, rehab, mental health, provider visits).
- Minimum 1 year of experience in assisted living, home health, or long‑term care (preferred).
- Licensed as an LPN with current and valid state license.
- Certified in basic CPR/Life Support.
- Proficiency with electronic medical records.
- Passionate about senior care and a strong resident advocate.
