Wellness Nurse; LPN

Position Summary

We are seeking a Wellness Nurse to monitor the health and wellness of our residents. The role includes ensuring overall well‑being, training and monitoring medication care managers, care associates, and personal care workers where permitted, and communicating effectively with resident support services and families.

Full‑time (8:00AM – 6:30PM, including a 30‑minute unpaid break). The schedule repeats every two weeks:
First week – Sun, Mon, Tue, Thu;
Second week – Mon, Tue, Thu, Sat.

Base rate: $33.00–$37.00 per hour. Shift differentials: $2.00 per hour for weekday evening, $2.50 per hour for weekday night, $4.00 per hour for all weekend shifts.

• Develop relationships with residents and their families by applying a consistent caregiver approach and putting the resident first.
• Complete observations and evaluations of residents for changes in clinical condition.
• Update or modify resident service plans as needed.
• Identify ongoing resident needs to maintain high levels of well‑being and ensure medical needs are met.
• Monitor each resident’s medication/treatment profile; ensure accurate administration, availability and documentation.
• Reconcile medication deliveries for accuracy and timeliness.
• Observe, evaluate, monitor, and follow up with residents who have experienced falls, accidents, or incidents.
• Collaborate with Assisted Living Manager, Memory Care Manager and Wellness Manager to analyze care and clinical outcomes.
• Assist in coordinating ancillary support services such as home health, hospice, outpatient rehabilitation, mental health and provider visits.

• Minimum of 1 year experience in assisted living, home health or long‑term care (preferred).
• Passion for senior care and a strong resident advocate.
• Current and valid state license as LPN.
• Current certification in basic CPR and life support.
• Experience with electronic medical records.
