Job Description

Open your home to provide an individual with intellectual and developmental disabilities a unique opportunity to live with a family unit. AFL living enables a person to live in a private home with a family and receive 24‑hour supports and habilitation. Easterseals PORT Health is seeking a person interested in becoming an Alternative Family Living (AFL) Provider in North Carolina.

Responsibilities
  • Provide a stable and caring home for a person with intellectual developmental disabilities and/or mental health challenges.
  • Offer 24‑hour supports and habilitation while they grow and integrate into the community.
  • Facilitate participation in day programs or other services as needed.
  • Collaborate with staff to develop and monitor daily living skill and independence goals.
  • Maintain a safe and supportive environment in line with agency policies.
Qualifications
  • Preferred experience as a CNA or DSP.
  • Empathetic and patient attitude toward individuals with disabilities.
  • Ability to live in a private home that meets program requirements.
  • Strong communication and team collaboration skills.
Contract Information

This is an opportunity for an independent contractor and is not an employment position.
