Gentiva is seeking an After hours RN Care Manager to provide compassionate hospice care outside standard business hours, ensuring seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours.
The RN assesses and coordinates patient and family needs during after-hours shifts, maintains primary clinical oversight, participates in IDG coordination, and supports chart reviews, plan of care updates, and regulatory readiness.
