Job Opportunity

This position is a part-time, hourly, non-exempt role.

Work Schedule:

every Saturday and Sunday 8am-5pm, 24 hours/week (Monday or Friday will be added to the weekend).

Location:

travels to homes and facilities between both of our Asheboro and High Point service areas (serving full 8 county territory).

This position provides personal care and home management tasks at patient's residence, under the supervision of a RN Case Manager.

Essential Duties Day to Day:

• Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility.
• Reports changes in patient's condition, concerns about patient and/or family to appropriate nursing staff in a timely fashion and participates in interdisciplinary team conferences as necessary.
• Works collaboratively with the IDG and other departments in achieving patient care goals.
• Documents patient care according to agency policy and procedures; documentation is clear, complete, concise, and timely.
• Responsible for professionally managing time, people, medical supplies, and technology.
• Establishes and adheres to a timeframe for patient visits in coordination with patients, families and facility staff members.
• Maintains a clean, safe and comfortable environment for the patient.
• Teaches and/or reinforces home safety, energy conservation and sanitation.
• Provides light housekeeping tasks.
• Assists with, or provides, meal preparation and related tasks.
• Demonstrates excellent communication skills and creates successful interpersonal relationships with patients and families.
• Complies with organizational nurse aide standards.
• Participates in staff training and education programs.
• Average of 6-7 visits a day

Qualifications

Education: High School or better.

Experience: 1 year:
Experience as a nurse aide (CNA)

Licenses & Certifications: Required: CNA, Auto Liability, Drivers License. Preferred: CHPNA
