Job OverviewThe Care Coordinator manages members’ Individual Service Plans in accordance with state law and contractual requirements. This role focuses on assessing, identifying, and coordinating non‑clinical program needs using standardized tools and established processes. The position supports members with chronic conditions, co‑morbidities, and/or disabilities through effective care coordination and collaboration with interdisciplinary teams.
Key Responsibilities- Manage members’ Individual Service Plans within the scope of the role.
- Conduct telephonic and/or in‑person assessments using standardized, pre‑defined tools.
- Identify and coordinate members with potential clinical healthcare needs with appropriate clinical and interdisciplinary teams (no clinical judgment required).
- Manage non‑clinical needs to support cost‑effective and efficient use of services.
- Collaborate with members, caregivers, families, and providers to establish short‑ and long‑term goals.
- Identify members who may benefit from expanded services and facilitate referrals.
- Bachelor’s degree (BA/BS) or equivalent combination of education and experience.
- Minimum of 1 year of experience working with the program population or in community‑based organizations.
- Bachelor’s degree in a healthcare‑related field preferred.
- Requirements may vary based on state law and contract.
- Ability to travel to worksites and other locations as needed.
