Contract Type: Travel Contract

Shift Type: Day shift (days; specific start/end times and specific days not provided)

Hours per Week: 32

• Assess patients’ functional mobility and design individualized physical therapy plans
• Deliver evidence-based interventions to improve strength, balance, and independence
• Document progress clearly and timely to support the interdisciplinary team
• Collaborate with nurses, OT, SLP, and physicians to optimize patient outcomes
• Educate patients and families on home programs and safe discharge planning
• Contribute to a positive, patient-centered culture at Givens Estates

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — ability to obtain NC licensure if not already licensed
• DPT degree preferred; new graduates are welcome to apply (years of experience: any)
• Comfort working in a Skilled Nursing Facility (SNF) setting
• Strong clinical reasoning, documentation skills, and teamwork mindset
• Current CPR/BLS certification (or willingness to obtain)

• Doctor of Physical Therapy (DPT) preferred
• Open to new graduates with DPT or equivalent qualification

• Clinical expertise in therapeutic exercise, gait training, transfers, and fall prevention
• Experience with SNF care plans, restorative programs, and skilled documentation
• Excellent communication skills with patients, families, and interdisciplinary staff
• Ability to prioritize caseload, manage time, and provide efficient care
• Familiarity with Medicare/rehab documentation expectations and regulatory standards
