Position

CAP Nurse / PT / Buncombe & Madison Counties – Flexible Hours. Regular schedule.

Responsibilities
  • Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.
  • Evaluate and document client programs.
  • Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies to obtain information to be incorporated into a social history that aids in planning for required services as appropriate to the individual plan of care.
  • Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team.
  • Coordinate services with RN to ensure quality care.
Qualifications
  • Graduation from an accredited School of Nursing.
  • RN certification (one year of nursing experience preferred). Ability to direct and carry out prescribed medical treatments and therapeutic programs.
  • Thorough knowledge of professional nursing theory and practice.
  • Valid NC driver’s license and clean driving record (condition of employment).
  • Personal auto liability insurance required if transporting clients in a private/personal vehicle.
  • Must meet all other conditions of employment related to criminal record screening, drug screening, reference checking, etc.
