Responsibilities
  • Provide 1:1 care for clients following their plan of care at all times while maintaining client dignity and independence
  • Administer medications, feedings, oxygen, and ostomy care as needed per the client’s plan of care and assessed results
  • Accurately document nursing actions and communicate with family, case manager, physicians, and document progress and outcomes for established goals, keeping them informed of changes in medical condition and needs
  • Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
  • Ensure all patient needs are met in accordance with physician instructions
Qualifications
  • LPN/LPT/LVN degree from an accredited college of nursing, current unrestricted registration and license in the state of practice and in the state of residence
  • Valid driver’s license, acceptable driving record, and proof of car insurance
