LPN; Home Health , HCHB

Overview

LPN needed in Ashland, KY. Home Health and HCHB experience REQUIRED. 13-week contract. Compensation: $35/hr.

Shift: 8:00 am – 5:00 pm. 40 hours guaranteed. On-call schedule: every 4 weeks. License Requirements: KY LPN or Compact.

Counties covered:
Boyd, Carter, Lawrence, Elliott, Greenup. Coverage across counties may vary by day.

• Licensed Practical Nurse (LPN) with KY license or Compact state.
• Experience in home health care; HCHB experience required.
• Previous experience with home care software (Homecare Homebase).

• Provide care for clients according to the client’s plan of care while maintaining dignity and independence.
• Monitor vital statistics and report abnormal findings.
• Administer medications, feedings, oxygen, and ostomy care as needed per plan of care and assessments of results.
• Complete routines established by other care professionals (PT, OT, Speech Therapists).
• Maintain infection control policies; report possible communicable diseases and ensure infection control procedures are implemented.
• Accurately document nursing actions, communication, progress, and outcomes for established goals.
• Order supplies, medications, and feedings as needed to ensure adequate stock.
• Ensure patient needs are met in accordance with physician instructions.
• Monitor incidents and ensure timely, accurate incident reporting with appropriate notification.
• Ensure medical equipment is maintained and report faulty equipment to DME for repair.

• Must pass background check and drug screen and have updated immunizations.
