Key responsibilities of the LPN/LVN – Travel include:

• Provide care for clients following the plan of care while maintaining dignity and independence
• Monitor client's vital statistics and report abnormal findings
• Administer medications, feedings, and oxygen as needed
• Complete routines established by PT, OT, and Speech Therapists
• Ensure infection control policies are maintained
• Accurately document nursing actions and progress
• Order supplies and medications to ensure adequate availability
• Ensure all patient needs are met per physician's instructions
• Monitor incidents involving individuals and complete reports timely
• Ensure medical equipment maintenance and report faults

Requirements of the LPN/LVN – Travel include:

• KY LPN or Compact license
• Previous experience with Home Care Home Base
• 1 year of home health experience
