Job Summary Responsible for collecting and analyzing patient data to formulate individualized care plans. Determines progress toward outcomes and coordinates care delivery. Promotes a safe practice environment and advocates mutual respect with patients, families, and healthcare providers.

Essential Functions
  • Conducts in-home assessments and develops, implements, and evaluates individualized care plans in partnership with patients, representatives, and caregivers.
  • Communicates significant clinical changes or findings to physicians and other healthcare practitioners involved in the plan of care.
  • Educates patients and families on disease management, medication administration, and preventative care.
  • Monitors vital signs, observes for changes in health condition, and provides wound care as needed.
  • Collaborates with healthcare professionals to ensure holistic and coordinated patient care.
  • Maintains accurate and timely medical records and documentation.
  • Supervises and instructs home health aides and student nurses as directed.
  • Takes on-call responsibilities during evenings, nights, holidays, and weekends on a rotating basis.
  • Performs other duties as assigned.
Education Requirement
  • Associate’s degree in nursing
  • Bachelor’s degree in nursing (BSN) is preferred
Experience Requirement
  • 0-2 years of experience in nursing, or a relevant area
  • An equivalent combination of education and experience may be considered.
  • All experience must be paid and in the same related field.
  • Part-time and PRN experience will be prorated based on hours worked per week.
  • Volunteer work and internships for academic credit are not counted.
Certifications & Licensures
  • Current and active Registered Nurse (RN) license, Basic Life Support (BLS)
