Family Care Coordinator / Case Manager

Salary Range: $44,000.00 To $48,000.00 Annually

KVC is seeking a Family Care Coordinator / Case Manager to work directly with families on a day-to-day basis while also managing and coordinating client care. The Family Care Coordinator / Case Manager will work with the Family Preservation and Reunification Services contracted by the state.

• Establish initial contact with families within 24 hours of referral to assess safety,
• Initiate safety planning, and connect families with resources to meet immediate needs;
• Conduct team meetings and maintain communication with the family and providers
• Facilitating the implementation of each family’s Action Plan
• Make frequent and consistent contact with DCBS personnel
• Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
