JOIN OUR FAMILY!As a Hospice and Home Care Nursing Assistant, you will provide personal care under the supervision of a nurse according to the patient's plan of care and collaborate with the trans‑disciplinary team in a variety of settings.
What You Will Do- Provide personal care and assist with ADLs according to the POC.
- Provide safe environment for patients.
- Safely ambulate/transfer patient as appropriate.
- Assist with passive and active ROM exercises as directed by the nurse.
- Prepare nutritional snacks as appropriate for the patient.
- Assist patient with pre‑poured medications under the nurse’s supervision (Home Care only).
- Perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
- Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
- Record activities and observations in clinical records and relevant documents at the time of visit.
- Complete all charting, timesheets and mileage logs as defined by agency policy and procedures.
- Attend and actively contribute to patient care conferences.
- Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in‑services as appropriate.
- Attend in‑services to obtain 3 education hours per quarter.
- Participate in the on‑call rotation as scheduled (Home Care only).
- Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.).
- Demonstrate positive customer‑service responsiveness according to agency standards.
- Be flexible to meet patient/family needs that require variable shifts and weekends.
- Participate in chart review quality improvement activities and other meetings as assigned.
- Participate in coordination of and communication with volunteer staff.
- Provide basic caregiving education to caregivers/family members, such as turning and positioning, transfer techniques and changing linens.
- Maintain transmission‑based precautions.
- Complete annual competency requirements.
- Must be able to read and write English proficiently.
- Must be a state‑tested nursing assistant in good standing.
- Equivalent education/experience may substitute for minimum qualifications except where legal requirements exist, such as a license, certification or registration.

- Provide an active auto insurance policy as a licensed driver in Ohio with the required coverage.
- Provide an active driver’s license.
- Ability to provide proof of eligibility to work in the United States.
- High level of integrity, ethics and professionalism.
