Scope of Role

Reporting to the Program Manager, Crisis Services, the Counselor, Crisis Services will provide counseling/crisis services and intake assessments to patients experiencing behavioral and emotional difficulties at local jails, Signature Health locations, client homes, and within the community. Services may be delivered in individual, group, or family settings and involve a team‑focused treatment plan approach to offer exceptional therapeutic intervention services.

Crisis Services
  • Provide crisis and intake assessments at local jails, client homes, and community settings.
  • Perform crisis prescreening and intervention to determine the appropriate level of care for the safety and welfare of the client and others, including provisional diagnoses and dispositions within available resources.
  • Obtain appropriate “Release of Information” documents at the time of assessment to enable two‑way communication with other organizations, providers, and contacts, acting as a liaison to facilitate client care and aftercare in an expedient and professional manner.
General
  • Conduct crisis intervention and assessment, and care coordination for both adults and children.
  • Actively engage in supervision with the Program Manager, Crisis Services.
  • Maintain client charting in accordance with Medicaid, JCAHO, ODADAS, ODMH, CARF, and other professional standards.
  • Prepare all documentation for discharge and aftercare referrals.
  • Assist in community outreach.
  • Act to understand and safeguard the welfare of others.
  • Consult with supervisor to address ethical and legal aspects of the organization.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform telehealth services as determined by Signature Health.
  • Complete compliance training provided by Signature Health.
  • Meet all productivity and documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Perform other duties as assigned.
Requirements Knowledge & Experience
  • Bachelor’s Degree required.
  • Valid, unencumbered LSW, LISW, LPC, LPCC, LIMFT, or MFT license in the state of Ohio required.
  • Working knowledge of the disease concept of chemical dependency.
  • Experience collaborating with other team members to optimize outcomes for clients.
  • Familiarity with state and federal healthcare regulations.
  • Awareness of community and state support resources for the population served.
Working Conditions
  • Work may be performed in a typical interior/office/clinical environment, client residences, and the community.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Possible exposure to blood‑borne pathogens while performing job duties.
  • Requires periods of sitting, standing, telephone, and computer work.
  • Must hear clients or patients in person, over the telephone, or via telehealth technology.
  • Must speak to clients or patients in person, over the telephone, or via telehealth technology.
  • Vision:
    Visual acuity adequate to read printed materials and computer screens.
  • Physical effort: occasional lifting and carrying items up to 15 pounds, unassisted.
  • Frequent bending, reaching, repetitive hand movements, standing, walking, squatting, and sitting, with some lifting, pushing, and pulling exerted regularly throughout the workday.
