General SummaryThe Home Health RN acts as a primary care manager for a caseload of patients. The RN’s role is to assess, plan, implement, and evaluate the plan of care coordinating with the patient's physician. The case manager functions in an assigned territory within Ashtabula County.

Current State of Ohio nursing license.
