We are a rapidly expanding behavioral health organization seeking a Psychiatric Nurse Practitioner (PMHNP) to support patients across affiliated long-term care and skilled nursing facilities in the greater Bronx and surrounding area. This is a full-time, Monday–Friday (daytime hours only), on-site position offering a highly organized clinical environment with strong operational and clinical support. - Conduct comprehensive psychiatric evaluations, including initial assessments and follow-up visits
- Diagnose and manage a wide range of behavioral health conditions across adult and geriatric populations
- Provide medication management, including prescribing and monitoring psychotropic medications
- Develop individualized treatment plans aligned with patient needs and facility coordination
- Utilize AI-supported tools to improve documentation efficiency and clinical workflow
- Collaborate with interdisciplinary teams including nursing staff, therapists, and medical providers
- Educate patients, families, and caregivers on diagnoses and treatment plans
- Monitor patient progress and adjust care plans as clinically appropriate
- Maintain accurate and timely documentation within EMR systems
- Coordinate care to ensure continuity and quality outcomes across settings
- Active and unrestricted Nurse Practitioner license in New York
- Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Experience working with adult or geriatric populations preferred
- Strong knowledge of psychopharmacology and treatment planning
- Comfortable working in a structured, high-efficiency care environment
