- Coordinate quarterly quality improvement meetings and analyze medication error reports to identify root causes and prevent recurrence
- Conduct nursing assessments and create, audit, and reconcile Medication Administration Records for accuracy and compliance
- Perform 60- and 90-day medication and nursing monitoring visits, updating care plans and assessments as needed
- Review and verify faxed medication orders, collaborating with healthcare providers to confirm accuracy and address contraindications
- Manage changes in Client medication administration, ensuring seamless transitions and timely communication with Caregivers and staff
- Lead training and competency evaluation for the medication administration program, including Caregiver redemonstrations and sign-off
- Delegate nursing tasks to Caregiving staff, ensuring proper training, supervision, and adherence to care standards
- Travel as needed to cover staff vacancies, perform Client evaluations, and maintain compliant nursing documentation
- Current LPN license in the state of Oregon required
- At least 4 years of management experience required
- Previous experience in home care, community health, or case management preferred
- Strong clinical assessment and case management skills
- Ability to train and mentor Caregivers effectively
- Excellent communication, organization, and documentation skills
- Working knowledge of electronic health records and care management systems (e.g., Epic, Well Sky, Axis Care, or Alaya Care)
- Must have access to a reliable, insured vehicle and be willing to travel up to 100%
