Role Responsibilities
  • Coordinate quarterly quality improvement meetings and analyze medication error reports to identify root causes and prevent recurrence
  • Conduct nursing assessments and create, audit, and reconcile Medication Administration Records for accuracy and compliance
  • Perform 60- and 90-day medication and nursing monitoring visits, updating care plans and assessments as needed
  • Review and verify faxed medication orders, collaborating with healthcare providers to confirm accuracy and address contraindications
  • Manage changes in Client medication administration, ensuring seamless transitions and timely communication with Caregivers and staff
  • Lead training and competency evaluation for the medication administration program, including Caregiver redemonstrations and sign-off
  • Delegate nursing tasks to Caregiving staff, ensuring proper training, supervision, and adherence to care standards
  • Travel as needed to cover staff vacancies, perform Client evaluations, and maintain compliant nursing documentation
Role Qualifications
  • Current LPN license in the state of Oregon required
  • At least 4 years of management experience required
  • Previous experience in home care, community health, or case management preferred
  • Strong clinical assessment and case management skills
  • Ability to train and mentor Caregivers effectively
  • Excellent communication, organization, and documentation skills
  • Working knowledge of electronic health records and care management systems (e.g., Epic, Well Sky, Axis Care, or Alaya Care)
  • Must have access to a reliable, insured vehicle and be willing to travel up to 100%
