** Clatsop Behavioral Healthcare has moved most positions to a 4 day 9 hour work week with paid 30 minute lunches to promote a better work life balance. Wages have also been increased for our valued current and future employees.
** Clatsop Behavioral Healthcare has an immediate opening for a full time Wraparound Family Partner on the Child and Family Team. The primary purpose of this position is to develop, coordinate, lead & facilitate youth & family teams to support the highest risk mentally ill youth receiving Oregon Health Plan. Coordinate Intensive Service Array mental health services at Clatsop Behavioral Health.

Service, support, and assist families to maintain youth in the home or to return to home and/or schools in community, rather than more restrictive out-of-home services such as hospitalization, psychiatric care or residential care.

This position works both as a wraparound family partner (75%) and providing case management for the Child & Family team (25%).  Approximately 25% of the time, this position will provide case management, care coordination, referrals & support in accessing services, individual & group skill building, and a variety of parenting & socialization activities for youth & families.
** Essential Functions/Major Responsibilities**:

** Wraparound Family Partner
*** Implement skills training to support parents/caregivers in respect to their youth’s mental health treatment goals in consultation with the primary therapist & the treatment team.
* Meet with parents/caregivers, as determined by applicable OARs & the Wraparound team.
* Develop & maintain therapeutic relationships with client & family.
* With other members of the Wraparound team, participate in assessing risk & completing safety plans for youth & families assigned to the Wraparound program.
* With the other members of the Wraparound team, assist in scheduling & collectively providing at least monthly child/family team meetings (CFTs); meet with youth’s caregivers 2-4 times per month, as indicated, to provide peer support to youth’s parents/caregivers.
* Assist the team in identifying needs, brainstorming potential strategies, developing action plans and assist in developing & monitoring family’s Wraparound plan of care.
* Hold meetings in non –traditional settings including: school, home or other community   facilities.
* Facilitate resolution of differing points of view.
* Maintain a strength-based approach in working with families, community partners, team members & staff.
* Serve as a crisis responder for wraparound parents/caregivers during working hours, responding for on-site crisis stabilization, as necessary & virtually by telecommunication, when indicated.
* Empower youth & family voice. Utilize ‘wraparound principles’ to create an environment in which all parties are heard & respected and have an equal role in developing & managing the plan of care.
* Assure that appropriate authorizations are in place, access & linkage to services is achieved, advocacy takes place as needed, and service effectiveness & identification of unmet needs are monitored by the team.
* Share relevant lived experience with parents/caregivers, as appropriate, to support the youth/family treatment goals.
* Facilitate individual & group skill building and parenting/socialization activities for families.
* Attend all mandatory meetings, participate in weekly individual and group supervision.
* While working with clients independently in the community, represent the agency in a professional manner at all times.
* Transport clients in agency vehicles only to facilitate client-directed services.
* Ensure client confidentiality is maintained and adhere to all ethical & professional standards.
