You will work under the Program Director and coordinate continuing care, recovery resources, and family education while ensuring compliant documentation and patient-centered care.
Ideal candidates have 1+ year healthcare experience, CPR certification, and preferably a degree in behavioral sciences or social work; state CM certification is a plus.
