St. Croix Hospice seeks a Registered Nurse to provide in-home hospice care under the direction of the RN Case Manager. You will perform comprehensive assessments, develop and revise Plans of Care, administer medications, and document patient status to support quality of life and comfort.
You will collaborate with an interdisciplinary team, educate families, and participate in on-call duties as required. A valid RN license and adherence to HIPAA are essential.
