Position Overview
The Home Health LPN provides skilled nursing care to patients in their homes across multiple counties. This role focuses on delivering high‑quality, patient‑centered home health services, supporting patients with ongoing medical needs, and ensuring safe, coordinated care while working independently in the field.
Schedule & Work Details- Weekly Hour Guarantee: 40 hours

Monday through Friday, 8:00 AM to 4:30 PM - On Call and Weekend Requirement:

Yes, reimbursed between patient visits from first patient to last patient - Pay Rate: $30-34 per hour
- Provide skilled nursing care to patients in their homes under physician orders.
- Perform routine clinical tasks including medication reconciliation, wound care, vital signs, and patient assessments.
- Accurately document care provided in the electronic medical record (EMR).
- Educate patients and caregivers on disease management, medications, and care plans.
- Communicate effectively with RNs, physicians, therapists, and the interdisciplinary care team.
- Maintain compliance with state regulations, agency policies, and infection control standards.
- Active LPN license
- Current CPR certification
- Ability to work independently in a home health environment
- Homecare Homebase is a plus
