As an LPN, you will provide direct nursing care to residents while collaborating with RNs, physicians, and the interdisciplinary care team. You'll play a key role in supporting rehabilitation goals, ensuring comfort, safety, and dignity in a long-term care setting.

• Administer medications and treatments as prescribed
• Monitor, document, and report changes in residents' health status, including vital signs
• Assist residents with activities of daily living (ADLs) as needed
• Collaborate with rehabilitation team members to support recovery goals
• Maintain accurate and timely documentation in compliance with facility policies and regulations
• Provide emotional support and education to residents and families
• Ensure a clean, safe environment by following infection control protocols

• Current and valid Licensed Practical Nurse (LPN) license
• Strong clinical skills, including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment
• Compassionate, patient-focused approach to care
• BLS/CPR certification required
