Transition Specialist/Job Coach

Instructs Pre Employment Transition classes to include, but not limited to, Social Skills, Workplace Readiness, Self Advocacy, Job Readiness, and/or Smart Work Ethics. Provides persons served/students with support that allows them to become active, involved, and valued members of their community. Provides support to individuals with intellectual disabilities to include, but not limited to, completing assessments, ensuring information for service billing is accurate, participate in and collaborate with program coordinator to meet funding source requirements;

assists with maintaining the premises; and assists with vehicle maintenance in coordination with supervisory staff. Responsible for providing training and support for individuals served, host site co-workers, families, area businesses and the community to ensure success during small group employment and/or a competitive employment. Is also responsible, as assigned, to provide extended services to individuals served who have gained and retained individual community employment.

Essential Duties and Responsibilities includes the following:

  • Prepare and Instruct Pre ETS sessions to school students with disabilities
  • Travel to assigned schools to provide Pre ETS training
  • Utilize virtual support services as warranted to provide services
  • Prepares accurate and timely documents to meet funding source requirements
  • Possess basic computer skills
  • Collaborate with Program Coordinator, QDDP, teachers, and other Transition Specialists in scheduling Pre ETS sessions and developing curriculum
  • Attend IEP and/or PCP meetings as needed
  • Provide support to individuals served participating in volunteer work, worksite tours, mock interviews, as well as, to individuals involved in paid employment with the Arc's integrated worksites.
  • Provides individualized support, training, and supervision for individuals served at the worksite or competitive employment which includes: skills training, building natural supports, identifying job accommodations, assistive technology, etc.
  • Perform specific job analysis, task analysis, situational assessments and job matching activities, when required.
  • Work with participants, employers, families, agency personnel, direct support professionals, Work Programs Coordinator, QDDPs, and other appropriate parties to problem solve issues related to training and employment.
  • Serve as a role model for students/persons served
  • Serve as an advocate for the persons served/students
  • Serve as an ambassador for the Arc of Madison County
  • Prepare and complete assessments and satisfaction surveys
  • Be punctual and in attendance as scheduled
  • Assists with community experiences, as needed, to include but not limited to, work site tours, volunteer opportunities, mock interviews, Special Olympics, or other community activities
  • Ensures information for billing of services is accurate
  • Assist, as needed, with maintaining staffing ratios to ensure individual safety and wellbeing
  • Provides, as needed, pre employment training to other individuals receiving services by the Arc of Madison County
  • Attends training, as appropriate, to become knowledgeable of strategies and current "best practices" in the field of supported employment and/or other areas.
  • Travel to worksites and access varying workspace as necessary
  • Participates in Person Centered Planning in collaboration with Service Coordination as needed, Prepares Assessments as warranted, etc., in accordance with funding source requirements
  • Assists in developing and delivering opportunities for persons served to achieve their life goals
  • Assists in maintaining funding source(s) and CARF certifications by ensuring standards and documentation are met
  • Assists with providing services through Community Waiver Program
  • Assists individuals with personal care as needed
  • Lifts, carries, or restrains individuals as needed
  • Other duties as assigned

Education Experience

High School diploma or GED is required. At least one year experience in day habilitation or residential program required. Other social services field may be substituted at the discretion of the Executive Director.
