Core Job FunctionsAssists in development and implementation of care plans based on level of need. Evaluate client's medical and health status, functional ability, social, emotional, and environmental factors related to illness, support system, home environment, formal and informal support in order to justify and meet the need for services. Serves as the liaison between the assessment process and effective delivery of direct services.
Monitors service delivery to clients to assure services are being provided as appropriate and effectively meets the client’s needs.
Demonstrates knowledge and skills necessary to identify each person's needs and provide care that contributes to outcomes sought by care plans. Understand the importance of client satisfaction and excellence. Rapport established with client/caregiver, physicians and providers to facilitate coordination of service. Manages provider requests for service changes based on the care plan and individual’s needs and updates authorization documentation timely and accurately.
Performs monthly contacts, quarterly site visits and reviews to provide effective case management. Adapts care plan in response to changes in an individual's circumstances or care plan variances. Assists clients with submitting financial documentation for eligibility and cost share determination.
Meets with supervisor at least monthly to review cases and the progress or problems associated with duties. Maintains knowledge of case management manuals and community resources. Compiles and submits reports as needed. Assists in development and revision of policies and procedures. Document all required and relevant case-related information timely and accurately. Provides timely and accurate documentation to providers to broker and manage services to prevent interruption of service(s).
