Community Access Group (CAG) Program Coordinator
CLASSIFICATIONS:
This position is an administrative, full-time position. This position is salary and FLSA non-exempt.
Executive Director PRIMARY
PURPOSE:
The primary duty is to oversee and manage the community-based services at Hope Haven:
Community Access Group (CAG) Services for adults with developmental disabilities. This position provides human services and case management across all the community-based programs. This position is principally responsible for supervision, program compliance and management, development of persons served, volunteer and community integration opportunities, ensuring that billing for services is maximized, community networking, and being a liaison between persons served and staff and between direct support and administrative staff.
This position oversees CAG Services by increasing opportunities for adults with developmental disabilities to participate in activities both in the community and at the facility.
ACCOUNTABILITIES
This position is accountable to the individuals that we support and their representatives, direct support staff, other administrative staff, and community partners.
Job Responsibilities PROGRAM MANAGEMENT 50%Manages and supervises all aspects of Community Access Group
- Ensures that all Case Coordinators and Direct Support Professionals working in the CAG program receive initial and ongoing/annual coaching and training consistent with Hope Haven's mission, core values as well as all state/regulatory requirements.
- Provides all supervision, performance evaluation and monitoring, and disciplinary actions for all assigned staff.
- Ensures that all program components meet all Federal, DBHDD, Regional Office, CARF, and agency P&P contract/waiver requirements.
- Arranges, leads, and participates in regular CAG programming staff meetings and ensures that minutes are taken and filed.
- Arranges and provides adequate program coverage at all times. (If necessary)
- Adheres to the programmatic and philosophical concepts in the provision of services.
- Assures that case coordinators and lead DSPs provide all necessary case management functions and provides this in their absence.
- Participates in planning processes and develops resources to ensure needed services for persons served and their families.
- Ensures and provides necessary orientation and facilitates Inservice training requirements for staff to promote their skill development.
- Acts as a primary resource person for staff to ensure the provision of the best possible service.
Provides oversight of case management for all persons served in the programs and maintains and follows record keeping requirements.
- Provides linkages of assigned persons served to other agencies by identifying needed services and supports.
- Ensures acceptable record keeping system and procedures necessary for all required reports and records for the CAG services.
- Facilitates outcomes by determining salient factors impacting persons served wellbeing and communicating them to appropriate individuals.
- Confers with other staff members and therapeutic disciplines in difficult cases to agree upon service delivery.
- Coordinates services, makes referrals, and follows up with other agencies as needed.
- Follows all requirements regarding record keeping including DBHDD, CARF, and agency P&P.
- Submits and completes all required records, reports, notes, and other written data in a timely manner.
- Participates in the development and implementation of CQI-RM activities and requirements.
Responsible for maximizing volunteer and integration opportunities for the people served, as well as developing quality facility activities.
- Continuously develops, seeks, and implements individual community sites to provide a variety of volunteer and integration opportunities to accommodate persons served choices and needs that facilitates their growth and assures work sites are safe, clean, and accessible.

- Ensures transportation is available for persons served when necessary for activities, trips, or employment.
Responsible for development, implementation, and evaluation of program outcomes.
- Stays knowledgeable and keeps abreast of program outcome measures relative to position responsibilities and program areas.
- Develops, implements, monitors, compiles, and evaluates results and outcomes.
- Responds appropriately to persons served satisfaction data to most effectively meet needs and requests.
As a member of the Administrative Team, performs other duties as assigned.
