Provides skilled nursing care in the home setting to patients ranging in age from adolescent through geriatric. Responsible for the overall management of an assigned caseload of patients, coordinating care with other nurses, multiple disciplines, physicians and community resources to achieve improved health and quality outcomes for patients. Practices in compliance with professional knowledge and standards of clinical practice and in accordance with policies and procedures of the agency, laws and regulations of Medicare, Medicaid and other third party payers.

Demonstrates professional growth and accountability. Reports to and is supported by the Piedmont Home Health Clinical Manager.

• Graduate from a nursing program Required
• Bachelors Degree Preferred

• 6 months of nursing experience in acute care medical/surgical nursing Required
• 1 year of nursing experience Preferred
• Nursing Experience in home health setting Preferred
• Prior case management experience Preferred

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
• BCLS - Basic Life Support Required
• Valid Georgia drivers license. Employee must comply with all applicable laws related to legally operating a vehicle Required
