Under the general supervision of an RN or physician, provides behavioral health and substance use disorder nursing services within a mobile behavioral health clinic in accordance with established nursing care plans and provider direction. Delivers nursing care to individuals with opioid use disorder (OUD), substance use disorders (SUD) and co-occurring mental health conditions. Responsibilities include conducting supporting nursing assessments, recording vital signs, monitoring overall response to medications, administering prescribed medications including injections and Medication for Opioid Use Disorder (MOUD), assisting with medication refills, and supporting medication adherence and safety.
Builds rapport and trust with individuals experiencing addiction through compassionate, nonjudgmental, trauma-informed care. Supports risk reduction, overdose prevention education, and client engagement to improve retention in treatment, particularly in community and rural settings. Participates in outreach and community education efforts to increase awareness of behavioral health and substance use disorder services. Demonstrates cultural humility, trauma-informed care, and a commitment to serving individuals and families with complex behavioral health and addiction needs.
Job Responsibilities and Performance Standards:
Support client nursing assessments, including vital signs, medical history, and medication reconciliation.
Collaborate with the multidisciplinary team to develop individualized care plans for clients.
Administer medications, including injectable medications, and treatments as prescribed by healthcare providers.
Performs various health screenings according to established guidelines.
Takes vital signs and performs other tests (such as glucose monitoring, pregnancy tests, UDS, etc.) as required by the prescriber following established guidelines.
Collects specimens for diagnostic testing according to established procedures.
Monitor clients for changes in condition and report any concerns to the healthcare team.
Provide education and support to clients and their families regarding treatment plans, medications, and self-care techniques.
Assist with crisis intervention and de-escalation techniques as needed.
Maintain accurate and up to date documentation of client care activities in the electronic medical record.
Coordinate with community resources and other healthcare providers to ensure continuity of care for patients.
Participate in multi-disciplinary staff meetings, training, and quality improvement initiatives.
Adhere to all regulatory and organizational policies and procedures.
Other duties assigned by supervising RN, Program Manager, or other members of leadership.
Minimum Qualifications:
Vocational/Technical degree from an accredited college.
Current Georgia license to practice as a Licensed Practical Nurse.
Certificates, Licenses, Registrations:
Active licensure to practice as a Licensed Practical Nurse in the state of Georgia.
