Our multidisciplinary team plays a critical role in advancing patient outcomes through comprehensive education, individualized transition planning, and long-term disease management. These efforts directlyimpactreduced readmissions, improved symptom control, and enhanced quality of life for patients living with heart failure. The heart failure nurse is central to this work, providing expert nursing care byutilizingthe nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum.
Practicing within the framework of organizational policies and procedures, the heart failure nursedemonstratesprofessional accountability, clinical leadership, and a commitment to ongoing growth. This roleis responsible for upholding standards of nursing practice, coordinating patient care activities, and collaborating with the interdisciplinary team to ensure safe, effective, and high-quality care. Through clinical expertise, coordination of care, and patient advocacy, the heart failure nurse contributes significantly to the success of the program and the delivery of exceptional outcomes for our patients.
ResponsibilitiesQualifications:
Minimum Education RequiredGraduate of a nursing program
Minimum Experience RequiredNew Graduates of a nursing program eligible
MinimumLicensure/Certification Required By Law
Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License. BLS certification required.
Additional QualificationsAdvanced certification in field of specialty, if applicable. Bachelor s degree preferred. Nursing Experience in Hospital Setting Preferred.
