Seeking Nurse Practitioners for locum tenens healthcare jobs delivering in‑home Health Risk Assessments to geriatric patients throughout Ohio. This flexible, part‑time clinical opportunity is ideal for independent Nurse Practitioners who value patient‑centered care, variety, and travel across Ohio urban and rural communities.

  • Location:

    Athens County, OH. Other openings throughout Ohio.
  • Employment Type:

    Locum Tenens, PRN/Part‑Time Nurse Practitioner.
  • Shifts per Week: 1 (minimum 4 full days per month; weekday coverage preferred, weekends a plus).
  • Shift Duration: 8‑hour field days (flexible start at 7:00 or 8:00AM).
  • Start Date:

    ASAP.
  • Assignment Duration:
    Ongoing, monthly scheduling.
Qualifications
  • Active, unrestricted Ohio Nurse Practitioner license (ANCC, AANP, or AACN Board Certification).
  • At least 1 year of Nurse Practitioner clinical experience (new graduates not eligible).
  • Accepted NP specialties: AGNP (Adult Geriatric Primary Care), FNP (Family Nurse Practitioner), ENP (Emergency Nurse Practitioner), ACNP (Acute Care Nurse Practitioner).
  • Ability to travel independently to patient homes, including rural and underserved Ohio areas.
  • Strong adaptability, communication, and documentation skills (same‑day electronic submission required).
  • Commitment to patient‑centered care and flexibility.
  • Business professional attire or dark‑colored scrubs required.
  • Must provide own patient bag with basic medical tools (tablet, PPE, badge; lab kits provided).
Job Responsibilities
  • Conduct annual in‑home Health Risk Assessments for geriatric patients (1:1 visits, no follow‑up).
  • Perform physical exams, medication reviews, and assess social/environmental factors.
  • Complete basic labs (A1C, fit tests; no blood draws).
  • Document and submit assessments using Matrix‑issued tablet (same‑day submission).
  • Travel daily to member homes within a 60‑mile radius (efficient route planning).
  • Adapt to diverse home environments and patient needs.
  • Maintain high quality scores and compliance with safety protocols.
  • Collaborate with the clinical services team for support and quality assurance.
