Licensed Practical Nurse - Supplemental Workforce; ETA-PN

Position Title:

Licensed Practical Nurse – Behavioral Health

You will provide direct nursing care and support to adults in a residential setting, helping them develop essential independent living and social skills. Your responsibilities include:

• Observing and supporting individuals with personal, grooming, hygiene and daily living needs.
• Assisting with medication management, including preparation, administration and record keeping.
• Maintaining a safe and supportive environment, de‑escalating crises, and modeling professional behavior.
• Transporting consumers to appointments and community activities as needed.
• Collaborating with interdisciplinary team members to develop, implement, and update individualized service and treatment plans.
• Recording progress, documenting incidents, and preparing related reports (e.g., medication logs, incident reports, treatment plan updates).
• Assisting lower‑level direct care staff and providing direction to new LPN staff as required.
• Performing clerical tasks such as answering phones, filing, and maintaining medication supplies.

Hourly position with a pay range of $27.56 per hour. Normal working hours are first shift (6:30 am–3:00 pm), second shift (2:30 pm–11:00 pm), and third shift (10:45 pm–6:45 am), and you may be required to work weekends and holidays.

• Licensed Practical Nurse, issued by the Ohio Board of Nursing.
• Proof of completion of an approved course in medication administration.
• For nurses who administer dialysis, Certification in IV Therapy (per Ohio law).
• Completion of a four‑week full‑time orientation covering all shifts and weekends.
