Great resident care starts with great assessments—and we're looking for an experienced MDS Nurse (RN/LVN/LPN) who believes people always come first to join our team. This role is responsible for coordinating resident assessments, ensuring accurate documentation, and supporting quality care planning while maintaining regulatory compliance. • Coordinate and complete MDS assessments in accordance with CMS requirements and facility policy
• Ensure timely and accurate MDS submission
• Collaborate with the interdisciplinary team to develop and update care plans; collect and validate supporting documentation from Nursing, Rehab, Social Services, Dietary, and Activities
• Monitor documentation to support PDPM and quality measures
• Participate in care plan meetings with residents and families
• Assist with survey readiness and audits
• Educate clinical staff on charting best practices that support accurate MDS coding and QMs.
• Review Quality Measures (QMs), 5-Star metrics, and facility dashboards; help drive performance improvement initiatives.

• Active nursing license in good standing
• MDS experience in skilled nursing or long-term care preferred
• Working knowledge of PDPM, QMs, Care Area Assessments, and CMS submission processes
• Strong organizational and documentation skills
• Passion for delivering excellent resident care
