Salary: $20.19 – $25.03 per hour based on experience, education, and/or licensure.

Overview

Integrated Services for Behavioral Health (ISBH) is a community‑mindful, forward‑thinking behavioral health organization serving Southeast and Central Ohio. We meet people in their homes and communities to connect them with needed resources.

We are seeking a Care Coordinator to provide service linkage and care coordination for children, youth, and families with significant behavioral health needs.

Responsibilities
  • Join with families to identify care coordination needs and services aligning with delivery standards and program outcomes to ensure best outcomes.
  • Work with families to define cultural factors that influence strengths, functioning, and interaction styles to ensure engagement and success in care planning.
  • Identify strengths of children, youth, and families to support healthy outcomes.
  • Coordinate family‑based services for children, youth, and families in home, school, and community.
  • Ensure services on care plans are appropriate, least restrictive, and meet safety and treatment needs.
  • Engage and build positive relationships with children, youth, and families in coordination with Child and Family Teams.
  • Develop collaborative partnerships with community resources to meet diverse needs.
  • Maintain documentation, participate in program evaluation, team and program planning meetings, cross‑systems training, and acquire knowledge of community resources.
  • Remain current with all required training, including high‑fidelity wrap‑around, MI, cultural humility, etc.
  • Perform all other duties as assigned.
Minimum Requirements
  • Experience providing services to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector or behavioral health field:
    • Three years with a high school diploma or equivalent; or
    • Two years with an associate degree or bachelor’s degree; or
    • One year with a master’s degree or higher.
  • Knowledge and experience in hi‑fidelity wrap‑around preferred (certification provided at time of employment).
  • Two years of experience in a coordinated supportive services or care‑coordination role preferred.
  • Experience working with individuals with autism spectrum disorders and developmental disabilities preferred.
  • Experience or knowledge in one or more of the following areas:
    • Family systems
    • Community systems and resources
    • Case management
    • Child and family counseling or therapy
    • Child protection
    • Child development
    • Cultural humility and responsiveness for managing complex cases
    • Work with children and families experiencing serious emotional disturbance, trauma, co‑occurring behavioral health disorders, and engagement with child‑serving systems.
    • Excellent organizational skills and ability to prioritize multiple tasks.
    • High degree of cultural awareness.
    • Experience with multi‑need families.
    • Broad knowledge of community service systems.
    • Willingness to participate in and lead cross‑systems care coordination.
    • Effective verbal and written communication skills.
    • Ability to operate in an internet‑based, automated office environment.
    • Valid driver’s license.
    • Capability to conduct video calls and meetings in an automated office environment.
    • Ability to maintain a minimum of 100 Mbps internet connection.
