Licensed Practical Nurse - LPN - Behavioral Health

Specialized care for individuals with Alzheimer's disease and other forms of dementia, including those experiencing behavioral disturbances.

• Facility Capacity: 24‑bed unit designed to provide a safe, structured, and supportive environment.
• Staffing Ratio: 1 caregiver for every 6 residents, ensuring personalized attention and quality care.
• Population Served:
  Exclusively geriatric residents, ages 50 and above.

Dayshift – 3×12 hour shifts one week and 3×12 hour shifts the next week, rotating weekends.

• Under the direction of an RN, contribute to the assessment of patients by collecting data for analysis by an RN.
• Perform plan‑of‑care interventions, including specimen collection and wound care. May administer medication. Document patient care.
• Assist RN with development of plan of care.
• Assist patients with performing activities of daily living – personal hygiene, elimination, nutrition, and ambulation.
• Monitor, document, and communicate patient’s status and reactions to interventions with RN, physician, patient, and families.
• Work as an advocate for the physical and emotional well‑being of the patient.

Applicants should have a current Tennessee LPN/LVN license or another compact state. Basic Life Support certification is required at the time of hire.
