Position: Nurse Practitioner (PRN) - In-Home Health Assessments

Overview

$1,500 Sign-On Bonus

We deliver personalized care to improve health, lower costs, and empower individuals to live their best lives. Our national network of clinicians meets members in their homes, helping older adults and other at‑risk individuals achieve better outcomes and identify chronic conditions that may otherwise go undiagnosed.

Responsibilities

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks to capture comprehensive insight that is often difficult to obtain in routine office visits. The Matrix Comprehensive Health Assessment (CHA) allows us to address multiple care gaps in a single encounter.

  • Conduct adult or geriatric assessments, including medical history, diagnosis and treatment, health education, physician and case‑management referrals, follow‑up documentation, and clear documentation according to Matrix guidelines and protocols.
  • Administer and order point‑of‑care and lab‑based testing as needed.
  • Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
  • Provide services at the member’s place of residence, which may be in their homes or skilled nursing facilities.
Benefits
  • Flexibility:
    Visits are scheduled based on your availability. We conduct assessments Sunday–Saturday from 8 am–7 pm; you can work during the day, evening, and/or weekend.
  • Competitive pay:
    Earn income based on the number of assessments completed, with a per‑assessment rate of $90–$115 depending on your state. Those completing a monthly average of 20–100 visits may earn $1,800–$11,500 per month.
  • Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
  • Education and support:
    Comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education.
  • Additional licensure support:
    Business need may allow you to obtain licensure and credentialing in neighboring or other states.
  • Full‑time opportunities: PRN providers can transition to full‑time roles supporting their home location or other regions, with a full‑time sign‑on bonus.
  • Insurance coverage:
    Bodily injury $100,000 per person / $300,000 per incident; property damage $50,000; collision and comprehensive deductibles $1,000 or less.
Qualifications
  • Master’s degree or equivalent experience and satisfactory completion of NP licensure.
  • Current NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority.
  • AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
  • Current BLS, ACLS, or CPR certification.
  • One year of NP experience preferred; new graduates encouraged to apply.
  • Valid state driver’s license and automobile insurance coverage for the state of residence.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills.
