Aveanna Healthcare is seeking a compassionate In Home Healthcare LVN to deliver one-on-one nursing care to patients in the comfort of their homes. In this role, you’ll perform assessments, administer medications and treatments, manage care plans, and monitor changes in patient conditions. You’ll collaborate closely with families and interdisciplinary teams to support optimal health outcomes. Responsibilities- Provide direct in-home nursing care to patients under physician orders
- Perform patient assessments and monitor vital signs and condition changes
- Administer medications, injections, and prescribed treatments
- Implement and update individualized care plans and document accurately in EMR
- Educate patients and families on disease management, medications, and home care
- Collaborate with interdisciplinary team members to coordinate care
- Maintain infection control and safety standards in the home setting
- Communicate changes in patient status promptly to supervisors and physicians
- Support pediatric and adult patients with chronic or complex medical needs
- Uphold Aveanna’s patient-centric, compassionate care standards
- Active LVN/LPN license in the state of practice
- Current BLS/CPR certification
- Pediatric experience or certification preferred
- Home health experience preferred
- Valid driver’s license and reliable transportation
- LVN nursing fundamentals
- Home health nursing
- Medication administration

- IV therapy and injections
- Patient assessment and monitoring
- Electronic medical records (EMR) documentation
