Telehealth Physician (Full-time, No On Call, M-F) – Primary Care Physician (Full-time Remote, Telehealth).

We’re looking for a Board Certified (or Eligible) Primary Care Physician to join our team and deliver high-quality, preventive-focused care through telehealth.

• Low-acuity, standardized protocols = predictable caseload and less stress
• Be part of a mission-driven team helping Medicare seniors live healthier, longer lives

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
• Create and manage personalized care plans for ongoing conditions
• Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
• Ensure thorough documentation in our EHR and compliance with quality measures
• Engage patients in preventive care to reduce avoidable complications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
• Collaborative, mission-driven mindset with excellent communication skills
