Social Services Assistant
Sandy Springs Center for Nursing and Healing is a 170‑bed facility located in Sandy Springs, GA. We are looking for a Social Services Assistant to support residents and their families.

Join our team to develop and deliver comprehensive social services that enhance the quality of life for residents and their families.
Responsibilities- Plan and implement a comprehensive social services program that offers counseling and other support services for residents and their families.
- Evaluate residents’ medical‑related social needs and formulate appropriate plans that fit into their total care plan.
- Coordinate and implement family meetings and create a forum for ongoing discussions among residents, their families/responsible parties, and the treatment team.
- Maintain residents’ rights at all times while liaising between all parties involved.
- Coordinate discharge planning and assist in creating an organized discharge plan for all residents.
- Ensure residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes their needs and rights.
- Bachelor’s Degree preferably in Social Work.
- Previous experience in long‑term care.
Competitive salary + full benefits.
ShiftMonday through Friday 9am‑5pm.
