Overview
Optum is seeking a Nurse Practitioner - House Calls to join our team in Fulton and Douglas Counties, GA. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.
In this role, you will provide annual clinical assessments to patients in the comfort of their homes. This preventive care helps identify and reduce health risks and coordinate appropriate follow-up care to improve health and well-being.

Primary Responsibilities- Conduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings including point-of-care testing (as appropriate)
- Identify diagnoses to be used in care management and active medical management for treatment
- Formulate a list of current and past medical conditions using clinical knowledge and assessment findings
- Communicate assessment findings to inform the member's PCP of potential gaps in care
- Identify urgent and emergent situations and intervene appropriately
- Educate members on topics such as disease processes, medications, and compliance

Required Qualifications- Active NP license or ability to obtain by start date. Licensure must be unencumbered and free of open or unresolved disciplinary actions
- NPs: if practicing autonomously or without formal supervision is permitted in your jurisdiction, ensure appropriate authorization; if not eligible at hire, begin processes within specified time frames
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or ability to obtain certification and/or NP license by start date
- Ability to meet physical requirements of the job, including moving a 30-pound bag, navigating stairs and various dwelling conditions, and performing physical assessments
- Driver's license and access to reliable transportation to complete home visit assessments
- 1+ year(s) of clinical experience in the highest level of education, clinical setting
- May be required to obtain additional licensure in other geographic areas
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
- Ability to travel throughout the state, up to 5% of the time; this may be eligible for additional compensation incentives
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
Compensation for this specialty generally ranges from $104,500 - $156,000
. Total cash compensation includes base pay and bonus and is based on factors including local labor markets, education, work experience, and performance. Optum complies with minimum wage laws. Benefits include a comprehensive package, incentive and recognition programs, equity stock purchase, and 401k contribution (eligibility applies).
