• Conduct thorough assessments of wounds and patients’ overall health status
• Develop and implement individualized care plans
• Perform wound care treatments including dressing changes and application of topical agents
• Monitor healing progress, watching for infection or complications
• Educate patients and families on wound care, prevention, and self‑management
• Collaborate with other healthcare professionals to coordinate care and optimize outcomes
• Accurately document assessments, treatments, and patient progress
• Provide care for all home health patients within scope of practice
• Stay current with best practices and advancements in wound care

• Active nursing license (LPN or RN) in the state of practice
• Wound care experience required (home health experience strongly preferred)
• Strong understanding of wound healing principles and infection prevention
• Familiarity with DME (Durable Medical Equipment) related to wound care
• Ability to travel between patient homes (reliable transportation required)
• Excellent communication and documentation skills
• Compassion, flexibility, and a commitment to high‑quality patient care
• Additional experience with pediatric patients, tube feeding, or medical‑surgical nursing is a plus
• Must be willing to drive through a designated territory and have wound care experience
