OverviewProvide direct nursing care to residents and supervise the day‑to‑day nursing activities performed by nursing assistants, ensuring compliance with current Federal, State, and Local standards and the Director of Nursing’s expectations.
Responsibilities Administrative Functions- Direct day‑to‑day functions of nursing assistants in accordance with regulations.
- Confirm compliance with written policies and procedures.
- Meet with nursing staff and support personnel to plan shift services, programs, and activities.
- Verify compliance with the Nursing Policies and Procedures Manual.
- Create written and oral reports regarding shift activities.
- Coordinate with resident services to maintain total care regimen.
- Ensure all nursing personnel comply with their job descriptions.
- Participate in development, maintenance, and implementation of the facility’s quality assurance program.
- Assist with government agency inspections.
- Review and update the resident’s discharge plan as required.
- Assist in planning the nursing portion of the discharge plan.
- Admit, transfer, and discharge residents as required.
- Complete accident/incident reports.
- Complete and file required forms/charts upon admission, transfer, and discharge.
- Encourage physicians to review and sign treatment plans and progress notes.
- Receive telephone orders from physicians and record them.
- Transcribe physician orders to resident charts, medication cards, and treatment plans.
- Chart nurses’ notes descriptively, reflecting care provided and resident response.
- Record new or changed diet orders and forward to the Dietary Department.
- Report discrepancies in orders or charting to the Nurse Supervisor.
- Complete transfer forms per established procedures.
- Document unauthorized discharges appropriately.
- Perform routine charting duties as required.
- Sign and date all entries in the resident’s medical record.
- Write nurses’ notes confirming the care plan is followed when administering care.
- Prepare and administer medication ordered by the physician.
- Responsible for IV medication/fluid administration.
- Verify resident identity before medication/treatment.
- Ensure medication is not given to the wrong resident.
- Confirm sufficient floor stock of medications, supplies, and equipment.
- Order prescribed medications, supplies, and equipment as necessary.
- Verify accuracy of narcotic records.
- Notify the Nurse Supervisor of drug or narcotic discrepancies.
- Review medication cards for completeness, accuracy, and stop order policies.
- Notify attending physician of automatic stop orders before last dosage.
- Immediately inform the Nurse Supervisor and attending physician of medication administration errors.
- Perform physical assessment (observation, inspection, palpation, percussion, auscultation).
- Deliver and maintain optimal resident care and comfort.
- Respond promptly to call lights and other resident needs.
- Make rounds with physicians as necessary.
- Consult with the resident’s physician regarding care, treatment, and rehabilitation.
- Review resident’s chart for treatments, medication orders, diets, etc.
- Check that prescribed treatments are properly administered by CNAs.
- Inform the attending physician of accidents or incidents.
- Notify physician and responsible party of changes in resident condition.
- Provide professional services such as catheterization, tube feeding, suction, dressing changes, colostomy, blood draws, massages, range of motion exercises, and end‑of‑life care.
- Monitor seriously ill residents.
- Review care plans daily to confirm appropriate care is rendered.
- Ensure CNAs are aware of and follow the resident’s care plan.
- Attend in‑service education programs.
- Know Standard Precautions, Exposure Control Plan, Fire Drill, and Evacuation Procedures.
- Maintain confidentiality of resident and facility records.
- Protect residents from neglect, mistreatment, and abuse.
- Protect resident personal property.
- Perform other duties as directed by supervisor or administrator.
- Registered Nurse with current state license.
- Excellent technical, assessment, and documentation skills.
- Good leadership qualities.
- Organized and detailed in work performance.
- Good communication skills, self‑discipline, and patience.
- Genuine caring for and interest in elderly and disabled people.
- Compliance with Residents Rights and Facility Policies.
- Ability to perform work tasks within physical demand requirements.
- Ability to perform essential duties as outlined above.
