Salary: $80,000-$85,000
Verovian Healthcare Recruitment Agency is seeking a compassionate and experienced Registered Nurse / Patient Care Manager to join a leading Home Health team in Atlanta, GA. This leadership role is designed for an RN who is passionate about delivering high-quality, patient-centered care in the comfort of patients' homes while guiding and supporting a multidisciplinary team.

Job Specification Responsibilities- Oversee and coordinate all aspects of patient care within the home health program
- Conduct comprehensive patient assessments and develop individualized care plans
- Supervise, mentor, and support nursing and clinical staff to ensure quality outcomes
- Collaborate with physicians, therapists, and other healthcare professionals to deliver coordinated care
- Monitor patient progress, adjust care plans as needed, and ensure compliance with regulatory standards
- Review clinical documentation for accuracy and compliance with agency and state guidelines
- Provide direct patient care as required, ensuring compassionate and evidence-based nursing practices
- Participate in performance improvement initiatives and contribute to policy development for home health services
- Current Georgia RN license (or eligibility for licensure)
- Bachelor of Science in Nursing (BSN) required;
Master's degree preferred - Minimum 2-3 years of nursing experience in home health, hospice, or community health
- Previous leadership, charge nurse, or case management experience preferred
- BLS certification required
- Strong knowledge of Medicare, Medicaid, and home health regulations
- Excellent communication, leadership, and organizational skills
