Ennoble Care is looking for a full-time, experienced Care Coordinator who will work out of a client’s senior‑living facility in Atlanta, Georgia, specifically in the Sandy Springs metro/suburb. The role is responsible for ensuring Ennoble Care provides high‑quality care services. The coordinator will collaborate with clinicians, staff, and patients to reach healthcare goals and maintain open communication. The position involves managing individual care providers, scheduling, and providing support for caregivers and families.
Responsibilities- Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care’s electronic medical record system.
- Maintain frequent contact with patients to provide care coordination, support, and manage compliance with care management programs to increase positive outcomes.
- Document all client communications (verbal or written) accurately.
- Communicate with Primary Care Clinician or designee regarding patient emergent needs and/or life‑threatening episodes to ensure comprehensive care plans are complete and accurate.
- Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues.
- Work with various cross‑organizational areas to meet the needs of Ennoble Care’s patients, their family members, and partner facilities.
- Become proficient with technology including secure email, telephone system, electronic medical records, etc.
- Adhere to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications.
- Demonstrate excellent customer service skills evidenced by positive customer and patient feedback.
- Contribute as a positive team member, supporting all members of the department in a productive and constructive manner.
- Utilize a computer, telephone, copy machine, and other office equipment as necessary.
- Comfortable speaking on the phone or in person for large amounts of the day.
- Compassionate and empathetic toward patients and families, consistently demonstrating exceptional customer service.
- Ability to take accurate notes to document each task in a timely manner.
- Ability to multitask between different patients and work streams while remaining organized and efficient.
- Ability to thrive in a fast‑paced environment.
- Must be able to work full‑time, Monday through Friday, daytime hours, at the client’s senior‑living facility in Atlanta, Georgia, specifically in the Sandy Springs area.
- Proficient in using a computer, including Outlook and other Microsoft Office programs.
- Basic knowledge of healthcare terms, conditions, roles, and basic care principles.
- Candidate must pass a drug screen, background check, have a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills continuously.
