Kaiser Permanente is seeking dedicated RN Case Managers to support care coordination, discharge planning, and continuity of care in the Atlanta area.

What You'll DoAs an RN Case Manager, you'll help patients access appropriate services and experience safe, coordinated transitions of care.
Responsibilities include:- Conduct comprehensive patient and caregiver assessments.
- Develop individualized case management plans and treatment goals.
- Coordinate discharge planning and patient transitions between healthcare settings.
- Assess medical necessity and appropriate levels of care.
- Review eligibility, benefits, referrals, and authorization requirements.
- Collaborate with physicians, nurses, social workers, and multidisciplinary teams.
- Identify barriers to care and connect patients with appropriate healthcare and community resources.
- Monitor patient progress and adjust care plans as needed.
Maintain accurate clinical documentation and support quality standards. - Promote patient safety, regulatory compliance, and continuity of care.
- Bachelor's degree in Nursing or a related field.
- Minimum 3 years of experience in nursing, case management, or a directly related field.
- Active Georgia Registered Nurse (RN) license required at hire.
- Strong clinical assessment, communication, and organizational skills.
- Ability to collaborate effectively with interdisciplinary healthcare teams.
- Basic Life Support (BLS) certification.
- Experience in inpatient or hospital case management.
- Experience with discharge planning, utilization review, or transitions of care.
- Knowledge of medical necessity, prior authorizations, and care coordination.
- Experience supporting patients with complex healthcare needs.
Hourly Pay: $47.40–$61.29 for the linked part-time position.
We're collecting interest for the following schedules:
- Full-Time: Subject to confirmed openings.
- Part-Time: Currently advertised 24-hour/week day-shift opportunity.
- Per Diem / PRN: Additional shifts may be available.
