RN Navigator - Day Surgery

The RN Navigator - Day Surgery at Kaiser Permanente is responsible for providing education and resources to facilitate informed decision-making and access to quality health care. This role is ideal for nurses who excel in patient advocacy and care coordination.

• Assist patients and families in understanding their diagnosis and treatment options.
• Perform assessment, symptom management, and case management for patients.
• Develop and facilitate individualized plans of care in consultation with treating physicians.
• Serve as a link between patients, healthcare providers, and support teams.
• Facilitate changes in the plan of care and necessary services.
• Perform care assessments and triage of new and ongoing patients.
• Facilitate appointments for consults, procedures, and support services.
• Standardize patient education materials and maintain a resource library.
• Deliver patient and family education throughout the treatment process.
• Monitor patient compliance with treatment plans and assist with referrals.
