Atlanta Pro Health case management has an immediate need for a Case Manager.

We are looking for a qualified Case Manager to provide person-centered case management to our Medicaid members and act as a key team member of our agency. This position requires the ability to communicate efficiently, an eye for detail, experience with conducting in-person and telephonic assessments, and a problem-solving attitude. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.

A certified case manager is a plus.

Responsibilities

  • Conduct thorough monthly and quarterly assessments to monitor care plans based on the individual care needs of each member
  • Oversee and coordinate service providers to ensure they comply accordingly to the members care plan
  • Review the SOURCE Waiver Program with the member and family/advocate
  • Maintain knowledge of policies and procedures of the state regulated SOURCE waiver program
  • Ensure SOURCE Waiver program requirements and objectives are met
  • Communicate the members status with doctors and other providers involved in the member’s care
  • Act as a member advocate and support them through the entire care process
  • Communicate effectively with the entire case management and clinical team
  • Accurately complete case notes, progress notes, document events and produce reports as per the company’s policy
  • Responsible for the maintenance of member files in the soft and hard chart

Skills

  • Georgia Medicaid EDWP Waiver experience is strongly preferred but will train the right candidate
  • Strong documentation and management skills
  • Computer literacy (Outlook, Microsoft Word and Google Drive)
  • Great communication and people skills
  • Excellent organizational and problem-solving skills
