Registered Nurse- Transition of Care

Position Summary
The Transition of Care Nurse provides consultative and coordinating functions in the transitioning of patients from the acute care facility to home or alternative care sites. This position works in conjunction with the Nursing and Medical/Case Management departments of the hospital and at the direction of the management of Atlanti Care LIFE Connection. The Transition of Care Nurse is also responsible for the coordination of care of all Atlanti Care LIFE Connection participants currently residing in an acute care setting.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

Coordinate transition of patients from acute care to home or alternative care sites in collaboration with Nursing, Medical/Case Management, and Atlanti Care LIFE Connection leadership.

Provide consultative and coordinating functions related to transitions of care to ensure seamless patient flow and appropriate discharge planning.

• EDUCATION: Graduate of an accredited school of Nursing required. Bachelor Degree in Nursing preferred.
• LICENSE: Current Licensure as a Registered Nurse in the State of New Jersey. Valid New Jersey Driver's license, Proof of Registration and Insurance required. BLS/HCP required.
• EXPERIENCE: Minimum 1 year with working with the frail or elderly required. Minimum 1 year experience as a Home Health or Long Term Care Nurse or equivalent experience required; 2 years of medical/surgical experience required. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

• Potential for exposure to the hazards and risk of the hospital environment, blood borne pathogens, including exposure to infectious disease, hazardous substances, and potential injury.
• Considerable reaching, stooping, kneeling, crouching; extensive standing, sitting and walking.
• Lifting approximately 25-50 pounds frequently; lifting approximately 50-100 pounds with assistance occasionally.
• Travel to company off-site locations is expected.
• Requires heavy mental stress demands.
• Ability to handle large numbers of interactions with multiple individuals compounded by constant interruptions.
