Full‑Time Outpatient Palliative Care Nurse Practitioner (NP)

We are seeking an experienced Palliative Care NP to join our growing team and make a profound impact in the lives of patients and families throughout Atlantic County, NJ. This is an exceptional opportunity to practice high‑quality outpatient palliative medicine in a collaborative, supportive environment focused on dignity, comfort, and continuity of care.

As an Outpatient Palliative Care Nurse Practitioner, you will play a critical role in improving the quality of life for patients facing serious or complex illness by providing compassionate symptom management, advance care planning, and care coordination.
Key Responsibilities- Assess and manage pain and other complex symptoms in the outpatient setting
- Lead compassionate goals‑of‑care and advance care planning conversations
- Support patients and families with hospice readiness and facilitate hospice referrals when appropriate
- Collaborate closely with physicians, specialists, home health, hospice teams, and community referral partners
- Participate in interdisciplinary team meetings and care conferences
- Coordinate seamless transitions across outpatient, hospital, home health, and hospice settings
- Serve as a trusted clinical resource and advocate for patients and families
- Compassionate, empathetic, and patient‑focused
- Skilled in complex conversations and relationship building
- Comfortable working independently while collaborating within a multidisciplinary team
- Passionate about delivering exceptional palliative care and improving patient quality of life
- Active Nurse Practitioner license
- National board certification
- Current state licensure and DEA registration (as applicable)
