Position SummaryCenter For Family Services is seeking a motivated, flexible, outgoing Case Manager for the Kinship Wraparound Program. The Case Manager is responsible for case management services for the Kinship program including, but not limited to: strong child/home assessment skills, investigating, verifying & documenting information gathered from home visits; keeping accurate records; providing home visits to all referred Kinship Care families; preparing reports;
making appropriate referrals to community services; maintaining strong working relationship with the court system, DCP&P and other community partners by acting as an inter‑agency and community liaison. Bilingual applicants encouraged to apply!
- Regard all information about clients as confidential
- Provide home visits to all referred Kinship Care families, maintaining respect for clients at all times
- Provide direct service in assisting eligible kinship caregivers with application for legal guardianship and eligibility for the Kinship Care Subsidy Program
- Complete and submit court assessment(s) to the appropriate Superior Court, Family Part
- Secure proper documentation to verify relationship, residence, age, and income
- Complete a home study, to include but not limited to: an interview with the caregiver, and all adult members of the household; a tour of the caregiver's home, (including the outside grounds); and, an observation and interview with the relative children
- Arrange for criminal history, domestic violence, and child abuse record background checks of all adults living in the caregiver's home
- Prepare written reports to the State documenting findings from the investigation
- Provide case management including problem‑solving, consultation, interviewing, coordination of services, appropriate community referral when indicated, advocacy and detailed record keeping of client progress, and preparing reports related to the provision of service
- Promote and recruit families to program
- To maintain records on all families referred
- Associate's Degree with two (2) years experience;
Bachelor's degree preferred - Successful experience working with individuals, groups, and families in a direct service situation
- Strong interviewing, assessment, and case management skills
- General knowledge of the resources available in the communities served by this program
- Ability to make independent decisions with guidance and supervision
- Possess successful experience providing direct services to adolescents
- Exhibit sensitivity in a multicultural work setting and an understanding in human relations
- Must have strong communication, writing, organization, and computer skills
- Current driver's license & good driving record of 3 years
